ROC Transitions works therapeutically with young people aged 16 to 18 leaving care who need extra support to acquire the skills needed to be independent and sustain their own tenancies.
We provide bespoke packages to help local authorities create effective pathway plans.
All Transitions homes are for solo young people with up to 17 hours daily support, including waking or sleeping nights, and 24/7 telephone support.
Our team are caring and supportive and apply boundaries consistently, often going the extra mile to help young people achieve their potential.
To smooth the transition to independent living, young people have the opportunity to move on to our supported housing charity ROC Solid, which offers tapered support in a house in the community.
Our team works with partners to promote:

• An understanding of actions and consequences to encourage behavioural change
• Citizenship and importance of having your say
• Self worth and confidence
• Positive links with the local community
• We have access to a range of privately-owned rented houses and can respond to applications promptly.
How we work
We provide bespoke packages for local authorities across the UK.
We work effectively and collaboratively with partners to deliver an outcomes-focused service, keeping the young person at the centre of everything we do. We provide support to help them achieve their goals and develop independent living skills to help them cope emotionally, financially and physically.
During their time in Transitions, each young person completes a practical workbook which evidences their unique journey into independence.
Referrals and admissions are part of the care planning processes, with rare exceptions of emergency placements. A robust risk assessment must be provided by the local authority and a placement planning meeting must take place within 48 hours of the placement starting.
As skills and resilience grow, hours are tapered so independence can be achieved. Young people who still need some support can apply to move into ROC Solid supported accommodation.
